Overview
Frozen shoulder (adhesive capsulitis) is a condition where the shoulder joint becomes stiff and painful, progressively limiting arm movement. The shoulder capsule (tissue surrounding the joint) tightens and thickens, restricting motion. Frozen shoulder typically progresses through three stages: freezing (increasing pain), frozen (maximum stiffness), and thawing (gradual improvement). Early diagnosis and aggressive physiotherapy prevent progression and minimize disability.
Common symptoms
- Gradual onset of shoulder pain
- Progressive loss of shoulder movement
- Difficulty reaching overhead or behind back
- Pain that worsens at night
- Stiffness that may last 1-3 years if untreated
The procedure
Early diagnosis and aggressive physiotherapy form the cornerstone of frozen shoulder treatment. Mr Mitra uses a staged approach: initial pain control (ice, anti-inflammatories, steroid injections), followed by structured physiotherapy to restore motion. For cases resistant to conservative treatment (3-6 months), arthroscopic capsular release may be recommended, followed by intensive physiotherapy.
Recovery
Most frozen shoulders respond to conservative treatment within 6-12 months, though resolution may take longer. Early physiotherapy is critical — delaying treatment increases stiffness and prolongs recovery. Steroid injections often provide relief and enable better physiotherapy participation. For surgical cases, recovery requires 3-4 months with intensive rehabilitation.
